Good Domain Host

This is what I need with a domain host:-

At least 10mb, preferably more
FTP
CGI/PHP
mySQL
No Banners (if impossible, user placable banner or button link)
No small bandwith limit

BTW, it's for a game site, so those gaming hosts are OK.
