You could signup for a register reseller account, like enom (http://www.enom.com/resellers/default.asp) and use their Register Rocket service, the client would pay for the domain. There are free hosting providors who offer free "reseller" accounts, you'd have to search

2) Unless you own a datacenter and a server, you will have to use someone elses services.

3) You can't, they cost money even for the registrars. You can get a reseller account, but that would require you to pre-pay per domain.
