I'm still wonder that there are companies which think about offering for $2.99?

If web proxy host means running cgi/php proxy script that turn server into web proxy available publicly then you need very powerful server.
In first instance when your traffic is low it will be not problem to find but once your site start to grow up and you receive more visitors you will need VPS or Dedicated server and it cost a lot more money.
I wish you good luck but whatever you find, once traffic start to raise up you will need more money to keep running your site.

I am willing to amend the features to only one domain, and 40GB bandwidth, and willing to pay a lump sum instead of $2.99 a month, but $36 for the year.

40GB is a very large amount of bandwidth. About 1 to 3 million page views. It will use a significant amount of CPU and RAM time for the server to generate these pages.

I recommend increasing your Budget to about $8/month if you want 40GB bandwidth for a proxy host. Or go VPS/DEDI.
