can i get some help please, i have tried with two diffrent forms and i cnat get it to work, what i sthe matter with this form

i have tried alot of things but i cant get teh form to work, i tried a pre installed script, then i installed one myself, then i tried remotly hosted, can someone help me with what is wrong with it? thanks

Try re-programming the apply button or get a different editor
