good php/mysql guestbook

I am looking for a good php/mysql guestbook that will accept name, email, website, aim, icq, msn, yahoo messengers, logs ip addresses. And also puts the date and time of message.

i have looked at hotscripts and resourceindex. I was just wondering if anyone uses a good one and could reccommend one.
