Cheap Host, Accept Money Order, Low Requirements (Looking for one)

Hi all. I am currently searching for a low cost host that offers only a few features (I don't need that many). All I need is:

-50mb or more space
-15 + gb data transfer
-high bandwidth
-php (new as possible)
-1 mySQL database
-ftp access
-sub-domain

I am willing to spend no more than 5 dollars per month. If anybody can offer me this, or knows when I can get this please dont hold back, Post now!.
