Sorry, that is a bit unclear isn't it.

This is the cost for a single mailbox. You can have as many forwards and aliases on the domain as you wish, however only one actual destination mailbox in this plan.

In our medium to large business plan, they pay more for a "domain administrator" account, and then add mailboxes to the domain - in the large business model, each additional mailbox can have up to 20M associated with it (10M standard, 5M intervals can be added). That's where the wording came from...
