You can get a fully qualified domain as a yourname.com.kg (org, net, et al, too) or yourname.com.ru (org, net, pp too) or US citizens can get a free domain under the .us domain. There are a few others that will give 3d level domains as well (AR comes to mind - if you're lucky and read Spanish fluently.)
If you settle for a sub-domain, anyone who owns a domain and has DNS control over it can give you one. Want to be yourname.birkitty.com, for example? No sweat!
Technically, WWW is a subdomain of your main domain, so whether you have that or not is up to you and whoever controls your DNS.
