Any good survey scripts?

Does any1 know of any good survey scripts that allow mi to set survey questions in many different forms like check box, radio buttons all these? And will actually give mi a result of the survey?

In a single poll, you want to put a radio button, checkbox and textbox? Kinda weird.

OR

In a single poll, you have the option to have a pure radio button poll, a pure checkbox poll, a pure textbox poll.

Kindly explain
