managed means there are some services included with the server itself (installation of things, server securing, etc) if you get an Unmanaged dedicated server it is up to you to setup, configure, and maintain your server

an unmanaged server means you rent one, the provider just takes care the network and hardware. you are the person to be in charge of software administrator
managed means your provider manages your server. They need to have staff enough to keep an eye on it 24/7, fix all the errors, including software and hardware, even before you know. There are some providers now bring you cheap "managed" server, but I dont call that "managed". It's some thing likes semi-managed. It shows that provider doesnt manage your server 24/7. Sometimes they can catch issues, but sometimes they dont know until you ring them. And they charge for some special things: reload OS, recompile kernel...
if you dont have experience with server management, you should go with managed server, but it costs more. Managing a server is not very easy work. It can bring you headache and your server is unstable, working in low performance

It all narrows down to what your host defines as "manage".

Agreed! You should contact the host of your choice and ask them what their managed services include. Some included more than others. Managed services that are worth anything will cost a pretty penny so be prepared
