transfering a domain name

ok maybe you people had heard of this question alot but I don't know how does it work.

I have a domain name with networksolutions, what form do I fill to change the NS and to change the "e-mail address and owner"?

Cause when I went to the site to change the NS my host told me that I am using the wrong form. when i thought it was right. so now I for got which one is it.

is there a fee for this? and do I need to fax the information to them? cause it seems that I do.
