I would recommend avadio.com. They only provide 100mb space/1gb bandwidth. But after 100 posts on the forums you can upgrade to plan 2, which will give you 200mb space/2gb bandwidth. It is hard to find a host that provides what you want. I would recommend hostmatrix.org, but there rules are too strict and i left the hosting there. I moved to lifelesspeople.com, which is by far the best host i have ever been to. They provide more than you want, but you have to do forum posting. And it is monthly. ut they support PHP4 and PHP5.
