This is pretty much what I need:

FTP (Preferable - if not browser upload)
5MB or so (Really does'nt matter)
Minimum ads - (no top banners)
CGI & SSI prefered
Access log (RAW log file) prefered
Telnet Account (Prefered)
Domain Host (or URL redirecting allowed)
Paid ads allowed
Reasonable amount of bandwith (3gigs/month+)
Links to adult sites allowed

Most of them (exept telnet) are is a real paid host.

If you contact MyAcen, I'm sure they'll find you a plan that has SSH for a reasonable price.

Yes, adult sites will severly limit your choices.

That is pretty cheap actually. But you would look at www.tera-byte.com
