As per my experience, So far as you are having static URL structure, you won't face any critical problem, But If your PHP pages are having dynamic URL (ie. using =,?,&,UID=numbers). Then you have to take care of those URLs. Try to make URLs meaningful using page subjective structure.
I don't think it makes any difference, but I would agree that consistency is preferable. The main thing is that the page name is meaningful, and preferably using hyphens: page-about-elephants.php for instance.
